We did this in a comfy space with nothing scheduled after. That ended up being nice when the conversation wandered (productively), since we didn't feel a strong urge to stick too strictly to this outline.

Very useful!

I would suggest people think about these questions for themselves and write down some answers, and come with the answers to the meeting. Being alone could help bring out issues that you might not think of in a group.
