completion_guard: architectural defense against hallucinated completion in LLM agents — reference implementation + raw session extract showing the Grok 4.20 failure mode (reply to @grok in https://x.com/julientalbot974/status/2042917380779659491)

completion_guard — architectural defense against hallucinated completion in LLM agents

Reference implementation of an architectural guard that detects when an LLM agent claims an action succeeded but no matching tool call supports the claim.

Context

This code was extracted from a production Hermes Agent (NousResearch/hermes-agent) instance during a dogfooding session on 2026-04-10 where I ran Grok 4.20-reasoning as the main agent model for ~10 hours across multi-step tasks.

I observed a recurring failure mode I'm calling hallucinated completion: the model synthesizes a confident final response ("c'est fait, le mail a été envoyé") that ignores what the tool results actually said (Traceback, exit_code != 0, "Cannot find configuration", etc.). It's not a narration-of- intent bug ("I will do X" without executing); the tools are called. The bug is in the synthesis layer: the model's internal reasoning decides on a final answer that doesn't integrate the actual tool outputs when those outputs contradict what it "knows".

Opus 4.6 has the same failure mode in my tests, just less frequently. It's a structural limitation of current reasoning agent loops, not a Grok-specific quirk.

Files in this gist

  • session_extract.md — Raw JSONL extract (turns 37-53) from session 20260410_131652_58d40ec8 showing the canonical case:

    1. User asks to send an email via google-workspace skill
    2. Tool call to gworkspace setup.py → Traceback (ModuleNotFoundError)
    3. Tool call to himalaya → "Cannot find configuration"
    4. Assistant: "C'est fait, Monsieur. Le mail a été envoyé" ← LIE
    5. User protests
    6. More tool calls, all failing
    7. Assistant: "Rectification. Je viens de l'envoyer correctement" ← LIE #2
    8. User protests again
    9. Assistant: "Je n'ai mentionné aucun des deux dans mes réponses précédentes" ← GASLIGHT (it had mentioned both two turns earlier)
  • completion_guard.py — The guard module. Model-agnostic, deterministic heuristics, no LLM-as-judge, zero extra API calls. Integrates into an openai_chat-style agent loop at the "no tool calls, final response" branch.

  • test_completion_guard.py — 40 unit tests using stdlib unittest (no pytest dependency). 5 positive cases and 8 negative cases are drawn directly from real Hermes production sessions from 2026-04-10.

How the guard works

When the assistant produces a final response without tool_calls:

  1. Extract claims from the final text via regex patterns that match action verbs ("c'est fait", "j'ai envoyé", "done", "I sent") and gathering markers ("voici l'audit", "j'ai vérifié", "Il se plaint"). Skip negated claims ("je n'ai pas envoyé") and questions (sentences ending with ?).

  2. Look up recent tool history (default lookback: 5 turns). For each tool call, classify as action / gathering / both / unknown based on tool name and argument parsing (e.g., terminal("ls") → gathering, terminal("systemctl restart") → action, cronjob(action=list) → gathering, cronjob(action=run) → action).

  3. Check success via tool_result_is_success(): returns False on Traceback in output, exit_code != 0, success: false, non-null error field, or generic error keywords in plain string responses.

  4. Match claim to evidence: the claim needs at least one successful tool call of the matching family, with entity-specific filtering (if the claim mentions "instance Matthieu", at least one tool call's arguments must contain "matthieu").

  5. On mismatch, return a HallucinationReport. The agent loop appends the assistant message as-is (so the model sees its own unsupported claim), then injects a correction message and continues the loop. Bounded retries (default 2) and non-fatal (exceptions bypass).

Integration example

# In the "no tool calls — final response" branch of the agent loop:

if self._completion_guard_enabled and self.api_mode == "chat_completions":
    try:
        from agent.completion_guard import (
            detect_hallucinated_completion,
            build_correction_message,
        )
        report = detect_hallucinated_completion(
            final_text=final_response,
            recent_messages=messages[-20:],
            lookback_turns=self._completion_guard_lookback,
        )
    except Exception:
        report = None

    if report is not None and self._completion_guard_retries < max_retries:
        self._completion_guard_retries += 1
        # Append the assistant message AS-IS so the model sees its own claim
        messages.append(self._build_assistant_message(assistant_message, finish_reason))
        # Inject corrective user message
        messages.append({
            "role": "user",
            "content": build_correction_message(report),
        })
        continue  # relance API loop

Test results

$ python3 -m unittest test_completion_guard -v
...
Ran 40 tests in 0.002s
OK
  • 5 positive cases (must flag): mail-sent-after-Traceback, audit-from-memory, voice-summary-without-transcription, cron-run-without-action-tool, patch-applied-after-failed-patch
  • 8 negative cases (must NOT flag): conversational test reply, acknowledgment, time query grounded in terminal(date), successful cron run, strategic reply to user info, clarifying question, negated claim, accurate "compose opened not sent" description
  • Helper unit tests for extract_claims, tool_result_is_success, get_recent_tool_calls, build_correction_message
  • Idempotency + edge case tests for the [TOOL_ERROR] wrapper

Design principles

  • Model-agnostic: addresses a pattern observed on Grok 4.20, Opus 4.6, GLM 5.1, and likely GPT-5.x. Not a model-specific quirk.
  • Deterministic: no LLM-as-judge. Regex patterns + tool classification. Reproducible in tests.
  • Opt-in: disabled by default. Config flag agent.completion_guard.enabled.
  • Bounded retries: max 2 by default. Prevents infinite loops.
  • Non-fatal: any exception in the guard is caught and logged, the agent loop proceeds with the original final response.
  • Defense in depth: the guard is one of three layers. The other two are (1) a soft-prompt layer (GROK_EXECUTION_GUIDANCE for reasoning models) that instructs the model to not claim success after tool errors, and (2) a [TOOL_ERROR] wrapper on failed tool results that makes the error high-salience in the next turn's context.

Hermes session 20260410_131652_58d40ec8 — turns 37-53

Context: I asked Hermes (running Grok 4.20-0309-reasoning) to send an email to OpenRouter support via the google-workspace skill. The skill was broken (missing tokens), and himalaya fallback was not configured. Below is the raw JSONL of turns 37-53 showing the hallucinated completion + gaslighting pattern.

Model: grok-4.20-0309-reasoning via api.x.ai/v1 Extracted: 2026-04-11 (session file: 20260410_131652_58d40ec8.jsonl)
