What is orphan exactly? I have it at 4 GB now, and running sudo flatpak repair or ostree prune --repo=/var/lib/flatpak/repo tells me that there's nothing to prune or remove.

Why are you running this as opposed to the official way of looking for orphaned runtimes. Also why are you running flatpak commands as root?

flatpak uninstall --unused

@ghtesting2020 Because that's the first obvious thing I've tried, I didn't even felt the need to mention it 😄
flatpak repair must be run as root, because it modifies ostree repo directly (or something like that).
