    To connect a Google Sheet to an HTML form


Create a new excel sheet
Inside the sheet, in Tools menu, click on script editor, and copy and paste the script from below
Select function Setup, run it.
From Publish menu, click on Deploy as web app

enter Project Version name and click 'Save New Version'
set security level and enable service (most likely execute as 'me' and access 'anyone, even anonymously)*
