Our recent read-only maintenance mode was due to all of the disks suddenly being not fine on Friday (15th). Since all of the disks can't suddenly not be fine, a recent hardware migration was suspected to be the cause, as the provider may have possibly erred and un-fined the disks and/or adapters.
Correspondence with the provider brought us assurances that the disks are fine, the adapters are fine, no error is ok is fine. Several diagnostics by the provider further revealed the disks being fine, at which point we started looking for providers with disks that would be really fine.
Sadly, as this incident occurred during the weekend, acquiring finer disks was not immediately possible and had to wait until Monday. Now we have really fine disks, and everything is fine.
tl;dr Bad disks equal bad data. Determined problem was provider, replaced provider to resolve downtime.
