Sub: Leave letter for dental treatments.
Respected Sir,
The purpose of writing this letter is to inform you that I will not be able to attend work upto January 21th, 2023. The reason for this is - recently, i had a small dental surgery (root canal), and doctor adviced to take rest for 3 weeks. For your reference, I have attached with the letter the doctor’s prescription. Moreover, I will make sure that I am able to resume the work after January 21 2023.
I do apologize for the short notice. Thank you for your consideration.
