Aether v5 — Binding Architecture for aiderDesktop + Companion (gaps + Phase A plan + FMEA + Z.AI kickoff). State on main as of 2026-05-18.

Phase A — FMEA (Failure Mode and Effects Analysis)

Purpose: Identify what can fail in the Phase A plan (PLAN.md), how severe each failure would be, and what mitigation we ship to prevent or detect it.

Method: Per-slice and cross-cutting failure modes. RPN = Severity × Occurrence × Detection (each 1–10; lower = better). High RPN (>120) requires explicit mitigation. Medium (60–120) needs a documented watchdog. Low (<60) is acceptable risk.
