A FLIPSIDE TO THE FLIPSIDE – A MOD-PACK MAKER'S AGREEMENT
A lot of the mods nowadays have requirements to add their mod into a modpack. But there are still some holdouts that require nothing for users to agree to should they use the mod in a modpack.
What I’m here to talk about is this:
Before I use a mod, I first want to let the modder know I have my own set of rules, to protect them and ensure that the users of my pack are geeting the best experiance ever.
Here is the set of rules I inform the prospective modder of mods in my pack I am going to follow:
I agree not to deliberately crash the game under any circumstances.
I agree that I will never include code or deliberately ignore a bug that makes a world unplayable, including a crash loop, serious exploits, or unusably low performance
I agree that I will strive to make my modpack as best as my capabilities allow
I agree that I will verify and report issues in a timely manner
I agree that I am using content made by you, not by me, so my wishes are secondary to the mod author whose knowledge of their mod enormously overshadows mine
I agree that under no circumstances will I hold the mod author responsible for the content in my pack
I agree that I will take full responsibility for damage caused by changes I make to the configs, and not blame them if I do something stupid.
I agree that all communication between me and you will remain confidential unless you choose to release it
I agree that I will be polite, courteous, and respectful of all everybody
I agree that these permissions I give to you are nonrevocable
I agree that I provide a reasonable certainty that my modpack will not adversely affect your modding enjoyment
I agree that I will strive to be a productive member of the community, and that I will refrain from criticism of the mod developers, regardless of the circumstances

I do believe I will Submit this to all the Mods I include in my pack. As a thank you to them.
