Useful Fabric server side mods
The vanilla command /execute in <dimension> run tp @s ~ ~ ~ lets you teleport between dimensions already.
I meant not like dimensions, I meant like other worlds. Like there’s a survival world, then there’s a creative world. How do i switch between them seamlessly without restarting the server?

I don't think there's a mod that lets you change your world folder without exiting/restarting.
