- Providers that use Epic or MyChart: in the app, Menu > My Records > COVID-19. Click on "QR Codes" which may be on the lower right of the screen.
- Others: Most pharmacy and hospital websites will have your COVID-19 vaccine information alongside other prescription or medical information in your account.
- Create an account at https://myirmobile.com/ if you haven't before. You will need your name and date of birth; if matching fails, follow the instructions for your state department of health to manually set you up.
- Click on "COVID-19: Get your proof of vaccination"
- Click on "Print QR Codes". Send the PDF.
- Download the appropriate app from https://docket.care/.
- Follow the registration instructions specific to your state.
- Get a QR code; screenshot it and send it over.
If all of the above have failed and you cannot obtain your records:
- Contact the healthcare provider who provided your last dose; they may be able to find your records and provide you with attestation
- Contact your primary care provider
- Contact your state's health department or immunization information system
- In the event you were vaccinated in a state other than that in which you reside, follow the instructions for that state
