Production incident (2026-05-13, datavant): A publish-app workflow ran through the full
extraction and query-parsing pipeline, then failed at the publish phase with "user disabled".
Mustafa T confirmed the triggering user's account was disabled in Keycloak. The entire extract
cost was wasted because no check validated IdP account status before the workflow started.
