I added the with-ruby-include to the last line. Not sure if it was necessary but it got me past the "LoadError no such file ruby_debug.so" problem.
If you are having the .so problem, I suggest uninstalling all copies of these gems from any gemset and starting over. Install them all into the gemset you will use and don't use the "global" gemset for these ruby-debug gems.
