Using new Chrome extension WiseStamp (http://pr-i.me/3G91). Best html signature extension around.

To use this html click the small HTML button on the right of the text box options. Just replace "full name" and "phone number" accordingly.

Also I went through the lower tabs "Social" and "IM" icons and removed all blank fields. http://pr-i.me/3G3V

Then just click apply and it will work automatically.
