This guide was likely given to you by someone who is running a multistate event and needs to verify that everyone is fully vaccinated against the Plague. This guide was written by someone who is in that same position, in order to collect all the individual states' worth of "how do I find this stuff" into one place.
You need to provide proof of full vaccination to (whomever sent you to this guide). Proof of a negative test is insufficient, and a "religious" exemption is likewise insufficient (see FAQ). (If you have a medical issue which prevents vaccination, please contact the requesting party for more information on what to do.) The hope is to use digital verification as an additional layer of validation on top of a simple paper card (we're not here for your "Maderna" crap) whenever possible.
Accordingly, the steps below for your state or vaccine platform will help you get a verifiable digital record (in many cases) or at least a digital record (in some others). This guide will be updated as new information is found; if you have updates you'd suggest, please feel free to DM the author on Twitter.
Please don't download a random sketchy app from the App Store or Play Store and import your vax card into that. Please. If your state or vaccine provider has partnered with someone officially, fine, but otherwise there's no need to share your information (which will then be used to market to you, as you provided it so it's not HIPAA... sorry) with J. Random Sketchy App Dev.
You should send the QR code and any information around it (i.e., a human-readable name and DOB, that kind of thing) to the event collection point. If you can't get a QR code following any of the instructions here, send a clear, unredacted photograph of the front and back of your CDC-printed vaccine card to the collection point.
Regardless of what you end up doing, expect to show your proof of vaccination again at the event, along with any on-site testing requirements.
That stinks. If you got your vaccine at a Big Pharmacy Chain, see that section for information about another way to get proof of vaccination. Alternately, if you either got your vaccine at or do your ordinary health work at a place with a major EHR system, check out Hospital EHR QR Codes for another option. (My EHR has the QR code for me even though I didn't get the vaccine there, because they validated my records against the state's immunization system; accordingly, this may work for you.)

California has its own state-specific portal through the California Department of Public Health. Fill out the requested information and send the entire resulting block of information (both the QR code and the data that surrounds it).
Colorado provides digital vaccine cards rather than QR codes through myColorado. This will likely get upgraded soon to provide QR codes, but in the meantime, provide the digital vaccine card.

New York created the Excelsior Pass program for access to COVID records. You will need to provide the "Excelsior Pass Plus" record, which includes immunization details, rather than the non-plus, which just provides a Boolean attestation.
You should be able to receive digital proof of vaccination, though not a QR code, from the NCDHHS Vaccine Portal.

Utah uses Docket for this.
The Virginia Department of Public Health will provide you with a verifiable vaccine record and QR code.
Go to WAVerify and enter your name and email/phone, along with a PIN (you only need the PIN for a minute). You'll get an email/text with a link; enter the PIN and save the resulting screenshot, and/or (on an iPhone) add it to Apple Health and Apple Wallet. Done!

To access your record in MyIRMobile:
- Go to https://myirmobile.com/ and register or create an account.
- If you've just registered or haven't linked your state immunization records, go through that process (it's a name and date of birth match; if you have issues, it should provide a number at which to contact your state department of health for assistance. That can happen; it happened to me, and WA DoH was extremely helpful).
- Click on "COVID-19: Get your Proof of Vaccination."
- Click on "Print QR Codes" and send the resulting PDF.
To access your record in Docket:
- Go to https://docket.care/.
- Download the appropriate app.
- Send the resulting QR code to the event.
If you got your vaccine at CVS, log into their CVS Care Portal and get your record/QR code there. Note that you didn't have to make an account at the time at which you got the shot; they'll have a shadow record for you, as verified by a very nice gentleman in that exact situation.
Rite-Aid partnered with CLEAR Health Pass (yep, the same people who let you cut the line at airport security) to provide a digital vaccine record.
Public information points to myWalgreens as being able to provide this information; however, this is as yet unverified.
Walmart and Sam's Club both say they can provide digital QR codes; however, this is as yet unverified.
If you got vaccinated at a healthcare provider which uses Epic's electronic health record product, and you have access to MyChart (the patient portal mobile app), you can get a QR code as follows:
- Open the app.
- Tap "Menu."
- Under "My Records," tap "COVID-19."
- Tap "QR codes" near the bottom of the screen (it may minimize itself into the lower-right hand corner).
- I have a medical issue which prevents vaccination. That stinks; I'm sorry. You'll need to contact the organizers of your event to see what needs to happen; likely you'll be able to provide evidence of testing, plus complying with any on-site testing programs they have.
- I have a religious exemption. That's not a thing. We don't have to believe that your interpretation of your sky man means that you don't have a duty to live in a society, so we won't. You're banned from the event, likely for life.
- I don't want to get vaccinated. Then you don't get to come to the event, or hang out with decent people. We will neither forgive nor forget your selfishness.
- I don't like your tone. Well, I don't like living in a society where one pseudo-political party aligned around greed and hatred of minorities has decided that their freedoms include infecting others with a deadly virus. Kindly fuck all the way off.
