OpenNet's country profiles appears to be the best list I can find of countries do currently have or in the past have had some level of filtering.
They also map by category.

Unfortunately it appears to be hard to get a definitive list of sites blocked by country. Wikipedia's list for China is excellent, but most other Wikipedia pages for other countries are not this complete or not presented as well.
