This is the Additional Charge Form from http://www.web-redesign.com/ for use as a boilerplate on Inkdit.com. The two column layout uses Zurb's foundation 4 grid (http://foundation.zurb.com/) so it works brilliantly on desktop and mobile.

What we do want is what’s best for both parties, now and in the future.

Kelly Goto Client Survey for use on Inkdit.com Use this html to create your own client survey as a boilerplate and/or online form on Inkdit. The form is from http://www.web-redesign.com/ and it is a resource of Web ReDesign 2.0 | Workflow that Works by Kelly Goto & Emily Cotler.

The Client Survey

(For the client to complete and return to the project manager)

Client input is the foundation upon which successful web sites are built. This survey will help you articulate and identify the overall goals of your site redesign, including specific questions regarding message, audience, content, look and feel, and functionality. Each key decision maker should fill out his or her own survey, and answer each of the questions in a thorough but brief and clear manner, and add any additional notes or comments at the end of the survey.
