pO2 < 60 mmHg or SpO2 <91% room air
pO2 > 50 mmHg and pH <7.35 (hypercapnic)
P/F ratio (pO2/FiO2) < 300
pO2 decrease or pCO2 increase by 10 mmHg from baseline
be careful overcalling postop acute resp. failure unless severe ie requiring non-routine aggressive measures
