It still works on all iOS devices and versions to this date. If it doesn't work for you either you did it wrong or your watch is not form an ECG-enabled region (the watch will have the ECG app by default).

If your watch was not from an ECG-enabled region (no ECG app), this won't work as your watch won't show ECG app until Apple enables it.

I’m getting a security error when restore is completed, because of that restore won’t work. Is it a must reset the iPhone? Did this on iPhone 11 Pro Max. BTW if I reset the iPhone will photos number count from the last restored photos or else start from 0.

I had a similar problem and then discovered there was a Management Profile blocking it that is part of setting up email and such for my company.

After removing that the restore worked without issue.

I think I may have also removed my PIN code beforehand, but I don't think that was the reason at the end of the day but worth trying that if you still get stuck.

I only remembered the Management Profile was there when trying to turn my PIN code off and it was greyed out.

This still works. I have just restored my XS Max, iOS 13.3 and followed the steps and it works. AW S4 (WOS 6.1) bought in London, I currently reside in East Africa, i couldn't use ECG before but now i can.

You can do the backup in iMazing and the edit of the backup, then export it to your iTunes backups folder and restore it using iTunes.

Yes, works, but in my case, I don't have any file to overwrite (but anyway I place the file on the folder anyway) and I need to reset the default configuration on the apple watch (delete all files and configuration, a factory reset) because many of the system apps don't works, after that everything works fine.

I have an iPhone 11 pro max 64gb with iOS 13.3.1 (beta) and an apple watch series 5 with watchOS 6.1.2.

You have supposed to Unpair the watch before starting, that’s probably why you needed to reset it to make it work.

Nop, I was needed to do both, first I unpair the watch, after I backup the iPhone, restore the backup and pair the watch, but many system apps don't work, I try to restart the watch (soft and hard reset) but the issue still there, my solution was to make a factory reset (Erase all content and settings).

I hope this can be helpful to someone.

I had the same issue when I’m trying this for the first time. When I give it shot for the second time I didn’t have enough space to restore. Therefore I had to check “erase phone” option from iMazing app. Once I done that, iPhone restored successfully.

I've tried pairing in a new iPhone, without the changes in the .plist edit and the ECG App is gone! I can't find it anywhere, tried the App Store inside the Watch or in the iPhone... I can't manage to install the app now, any suggests?

Reset watch & try it again

Reset several times, same results. Set up as new Watch, restored from backup, etc. Nothing worked. Still ECG app missing.
Even paired the watch to my wife's iPhone and ECG app is still missing. Anything else to test?

hi, following the steps, i have activated ecg from the health app for my iphone. For the watch, since i updated from watchOS 6.1 to watchOS 6.2.1, that’s when ecg app disappeared. My seller said the update made the ecg app dissapeared.

In the Watch, opened App Store app, search “Apple”, first app was Apple Store, click on Developer “Apple”, scroll thru all Apple’s apps, ECG is not even listed for reinstall.. It’s like my watch is not from ECG region... but it is!!

yes..exactly.. some tweak or plist file that works on ios13.4.1 . is what we need..i wonder that why apple did not activate it all over the world.. naturally ecg attraction is for those who know what it is all about..i mean health professional s..i m doctor and i purchased it for this main attraction.. in my country watch is too costly.. understand apple..

I can confirm this works for my Apple Watch series 4 (bought in México where it hasn't been activated yet) and an iPhone 11 Pro MAX. I have ECG app working, thanks a lot!

Steps I've followed:

  • Unpair Apple Watch from iPhone
  • Backup iPhone with iMazing
  • Edit iPhone backup with this file
  • Restore backup [Erase target device option checked (probably not needed... idk)] and wait until it automatically reboots the iPhone
  • Pair Apple Watch again and select as new device, don't restore from backup

Whole process took around 2 hours for me (probably a bit more) but it will depend on the information amount you have on your iPhone. Totally worth it and I can't believe Mexican Health authorities haven't approved this yet... but I don't care anymore :)

iOS 13.5 and watchOS 6.2.5

Mine worked initially following the steps about a month ago, then updated watch OS and ECG app disappeared on watch but stayed on phone.
Just updated to latests Watch OS and ECG app came back on watch. Go figure.

iPhone XR, Apple Watch 4 bought in Mexico
Works on the first developer beta of both iOS 14 and watchOS 7; was kind of a mess though. The first time the iPhone was stuck in a boot loop. Second time I disabled the SIM PIN, deleted all data before restoring and also deleted two com.apple.private.***.plist in the backup, one talking about age gating and the other about feature availability having been downloaded in the last 24 hours. Idk if it was one of those or a combination of them but it’s now working without problems

This fix has persisted now my iPhone 11 Pro Max is running iOS 14 developer beta 2 and my Apple Watch Series 4 (Nike) running watchOS 6.2.8.

Hola, aun funciona en el 2020 con la version de ios 13.6, imazing al inicio no me dejaba restaurar el telefono me salia un error como el subió alguien en los comentarios anteriores, para resolver eso fue simple desactivar el touch id y el código de desbloqueo y ya tengo activado la función de ECG. Espero que a alguien le funcione.

I confirm this file work for me. iOS 13.7, watch OS 6.2.8 LTE

Follow these steps:

  1. Unpair apple watch without cancel the subscription with your carrier (in case of LTE version)

  2. Download and backup with iMazing

  3. Edit the backup then Goto HomeDomain > Library > Preferences to choose com.apple.private.health.heart-rhythm.plist, right click then chooseCopy to 'Preferences' and point to downloaded file above

  4. Go back to the backup then restore the backup to your phone (Don't check Erase options, this will affected your e-sim activation)

  5. After restored. Pair and setup apple watch as a new device. That's all.

These steps takes me 30 minutes in total.

After patching PLIST file, Health app says: "open the ECG app on your Apple watch to take a recording" and subtext says: "Enabled on Mitja's Apple watch" and no birth date required as before patching PLIST file.

If the health app is telling you it’s enabled then probably it’s just a matter of finding it in the App Store. Try searching from another app by Apple, like Music, Timer, Noise, etc and inside the app page go to developer and tap Apple. It’ll show you all apps available

That’s very strange, I’m out of ideas :/
Personally, I’d start from scratch.
When I did it I found two com.apple.private.***.plist in the backup, one talking about age gating and the other about feature availability having been downloaded in the last 24 hours and I deleted them but idk if that did something. Also checked “erase target device” when restoring with imazing

Worked this when I was on iOS 13 & WatchOS 6. It also worked when I upgraded to iOS 14 & WatchOS 7. I reset Apple Watch due to poor battery life & not recording any data (heart rate, ECG, step count etc). After I reset Apple watch WatchOS ECG app says “setup on iPhone” even though I have already setup on iPhone & have ECG records also. I don’t know how to reset ECG to get to setup page on iPhone.

The original .plist is the one before you overwrite the .plist file above. For me, because I compared the original file, to get it working again I just removed these 2 lines:

<key>HKElectrocardiogramOnboardingCompleted</key>
<integer>3</integer>

And restored it back to my iPhone. It is now working like before I messed up with the .plist file, ECG app showing up in my Apple Watch but I just can’t activate it.

You can edit the .plist file with Xcode or with TextEdit, as long as you save it as .plist and not .plist.txt when you overwrite the file using iMazing or any other software.

The same with you, my phone is also 64 GB.
In order to avoid crashes and data corruption, please make a backup of your phone!!!
I left more than 20GB of remaining capacity in my phone, and about 20 minutes.

When I first used iMazing to restore my phone. It has crashed at this time, so I use DFU mode to restore the phone.
When I write the activation file to the phone for the second time, it's successful. 😄

Failproof guide:

  1. Prepare:
    a) Watch app > unpair Apple Watch
    b) Settings app > iCloud > Backup > update backup
    c) disable iCloud

  2. Restore:
    a) download latest IPSW
    b) select iPhone in Finder (iTunes)
    c) press "Option" key and "Restore" button
    d) select downloaded IPSW file.

  3. Setup:
    a) setup clean iPhone without iCloud
    b) select iPhone in Finder (iTunes)
    c) select "Encrypt Backup" and set Backup password.
    d) open Health app, fill in data, add 100 "Steps".

  4. iMazing Plist Injection:
    a) select your iPhone in iMazing, right click, select "Back Up" (don't allow system access, then give Backup password).
    b) select your iPhone in iMazing, right click, select "Create an Editable Copy of this Backup"
    c) select an Editable copy of your iPhone in iMazing > File System > HomeDomain > Library > Preferences, then drag and drop Health Plist to this folder, check if it's actually added before push (!!!)
    d) select an Editable copy of your iPhone in iMazing, right click, select "Restore this Backup to device"

  5. ECG Setup:
    a) open Health app, check if ECG data is enabled/exist (it should show a list with zero records, rather than introduction)
    b) add iCloud to iPhone
    c) open Health app, select Show all Health data, wait until it's loaded from iCloud.
    d) pair Apple Watch, clean setup without iCloud.

Yes, my Apple Watch is from non supported country. It stopped checking country origin around iOS 13.4

Awesome! Will give it a go later! I’m on Windows, but should be the same! Do you know if you have to redo the hack after each new watchos/iso update, or it’ll continue to work? Once again thank you for your reply! Very much appreciated…

a) ECG data is now linked to your iCloud account / Health app, so if you setup a new iPhone (even not from iCloud backup) - it's going to be automatically activated (no need iMazing or anything), since Health app has ECG data.
b) if your want to switch to another iCloud account (permanently), then when logging out keep the Health data, then merge it with a new iCloud.

I started the restore operation at 9:30 pm yesterday and almost 12 hours later the restore is not finished yet (it's 9:16 now). Is it normal for restore to take that long??? I checked the iPhone console window. The app seems to be working but the wait time is driving me nuts.

Should I interrupt the process? How do I interrupt it?

If you are on Big Sur make sure that you're using the last version of iMazing — they added MacOS Big Sur support only in 2.13

I confirm this file work for me. iOS 13.7, watch OS 6.2.8 LTE

Follow these steps:

  1. Unpair apple watch without cancel the subscription with your carrier (in case of LTE version)
  2. Download and backup with iMazing
  3. Edit the backup then Goto HomeDomain > Library > Preferences to choose com.apple.private.health.heart-rhythm.plist, right click then chooseCopy to 'Preferences' and point to downloaded file above
  4. Go back to the backup then restore the backup to your phone (Don't check Erase options, this will affected your e-sim activation)
  5. After restored. Pair and setup apple watch as a new device. That's all.

These steps takes me 30 minutes in total.

How did you edit the backup? I can't get it done, I right-click on preferences, select the file... but it has no effect, it's not coping the file

I activate my ecg with this metod and today after update to ios 14.3 and 7.2 on watch ECG stop working and tell me in this region is not avalible.Now this metod no more work. Any ideas? btw i try to restore but no lucky

iOS 14.3 uses ECG 2.0, original key no longer works, even if Health app already has ECG data.

After update to iOS14.3, can still using the method changing the value in the plist to activate the function of ECG?
I try to use the backup-method, change the value of HKElectrocardiogramFirstRecordingCompleted in the com.apple.private.health.heart-rhythm.plist to True, but it doesn't work.

Yes exactly this is what happening, App is present in the watch but when I try to activate it from mobile at that time it shows that error.

And my ecg now work after beta install but not properly. I start ecg app but when I place finger on crown nothing happens. Any ideas why? And one more thing. In health app when push button update ecg I see message “ Not work in my region “
