The substance of this request is not supported in the DMCA:

This is not necessary and may cause a miscommunication. Best to keep things as simple as possible:

Removed "Please send me a prompt response at the address noted below indicating the actions you have taken to resolve this matter." - it's not required by DMCA. Best to keep things simple.

Thanks for spending some time improving this! All of your changes are really valuable, so I've folded them back into the original and have given you credit.
This is a helpful resource. However the bit about "a cancellation message as specified in RFC 1036" only applies to Usenet. It shouldn't be in a generic document. so in this version here I have removed it from Jason's original.
