This is a helpful resource. However the bit about "a cancellation message as specified in RFC 1036" only applies to Usenet. It shouldn't be in a generic document. so in this version here I have removed it from Jason's original.

Actually that whole paragraph could use a bit of editing.

This bit because only applies to Usenet:

I request that you immediately issue a cancellation message as specified in RFC 1036 for the specified postings

The substance of this request is not supported in the DMCA:

and prevent the infringer, who is identified by their URL, from posting the infringing material to your servers in the future.

This is not necessary and may cause a miscommunication. Best to keep things as simple as possible:

Please be avised that the law requires you, as a service provider, to "expeditiously remove or disable access to" the infringing material upon receiving this notice. Noncompliance may result in a loss of immunity for liability under the DMCA.

Changes per @plagiarismtoday advice:

Added contact address and phone number.

Added pointer to http://www.plagiarismtoday.com/stock-letters/

Removed "Please send me a prompt response at the address noted below indicating the actions you have taken to resolve this matter." - it's not required by DMCA. Best to keep things simple.
